報(bào)名現(xiàn)場(chǎng)審核地點(diǎn)
各考生進(jìn)行現(xiàn)場(chǎng)審核要注意自己工作單位所屬的行政區(qū)劃，也就是看自己工作單位的《醫(yī)療機(jī)構(gòu)許可證》下方發(fā)證機(jī)關(guān)的簽章是何機(jī)構(gòu)就去何機(jī)構(gòu)進(jìn)行現(xiàn)場(chǎng)審核確認(rèn)，如發(fā)證機(jī)關(guān)為“云南省衛(wèi)生廳”或“中央駐昆醫(yī)療機(jī)構(gòu)”由工作單位出具委托函，說(shuō)明情況后到昆明考點(diǎn)報(bào)名；發(fā)證機(jī)關(guān)為“昆明市衛(wèi)生局”由工作單位統(tǒng)一組織，收集所有人員報(bào)名材料后報(bào)昆明考點(diǎn)，如單位不統(tǒng)一組織的，由單位出具本單位所需報(bào)名人員名單，并附授權(quán)書由考生自行攜帶至昆明考點(diǎn)進(jìn)行統(tǒng)一報(bào)名。
